Job description _Community Health Extension Worker at Rafiki wa Maendeleo Trust in Rarieda District - Kenya Jobs, Careers and Vacancies_ *COMMUNITY HEALTH EXTENSION WORKER* Rafiki wa Maendeleo Trust is an organization operating in rural Rarieda District. The organization began as a grass-root CBO in 2003 to assist orphaned children and youth through community empowerment. Rafiki wa Maendeleo Trust has since developed into a dynamic organization, focused on empowerment and development, both within the community and the organization. We are currently seeking to recruit talented, progressive and team-oriented individual to fill a position of Community Health Extension Workers, based in Rarieda District, Kenya. Mission: Empower people and build their capacities to create positive change for children and youth within their community. *Major Duties and Responsibilities* _Child Level_ * Carry out nutritional assessments for OVCs * Monitor health of malnourished OVCs on a supplemental feeding programme * Make referrals and linkages for children to local clinics for health care and disease prevention. * Educate CHWs on the need to make referrals to support survivors of sexual and/or physical abuse. * Provide support to medical personnel in field medical examinations _Caregiver/Family Level_ * Provide nutritional counseling to those families where low BMIs in children were detected * Refer families whose OVCs are on supplemental feeding to the internal agricultural staff to improve their food security. * Provide group counseling to families and caregivers on nutrition, diet and food preparation techniques, monitoring children’s health, how to talk to children about abstinence and safe sexual behaviors and support healthy life decisions. _Community & Systems Level_ * Co-facilitate health and psychosocial support training programmes. * Co-facilitate monthly training of CHWs. * Mentor CHWs in their field work, data collection and management. Monitor their progress and results to ensure effective delivery of disease prevention messages and health clinic referrals. * Provide back-up support at local health facilities _Internal Administration_ * Provide quality and timely delivery of all project reports, in their agreed format. * Maintain follow-up system to ensure efficient processes * Maintain and keep up to date and accurate field records and reports of activities and beneficiaries according to the organization’s M&E and reporting requirements. * Actively maintain the internal referral system to ensure beneficiaries are provided with maximum support from the organization’s various expertise *Other Duties and Responsibilities* * Actively participate in the project’s Saturday Psychosocial Support for orphaned and vulnerable children. * Any other duties and responsibilities, which may be assigned by the management from time to time *Knowledge, Skills and Abilities:* * Level of education: At least a Diploma in Nursing, Public/Environmental Health or any Health related field. People with medical Health background are encouraged to apply. * Specialized Training/Professional Qualifications.
